Cape Fox Corporation (CFC) Shareholders are encouraged to update their contact information and direct deposit through myCapeFox.com. All other forms and share transfer requests can be downloaded in the forms section and returned to the Corporation for processing.

Return the completed paper form to Cape Fox Corporation Shareholder Services in person at:  2851 S Tongass Hwy, Ketchikan, AK 99901 or via USPS at PO Box 8558, Ketchikan, AK 99901.

Address Change Form

To update your address, complete the Address Change Form and return to CFC. All future mail from CFC will be sent to the new address once processed.

Affidavit of Name Change Form

If your name has changed due to marriage or any other legal reason, you must complete an Affidavit of Name Change Form. The affidavit MUST be signed in the presence of a Notary Public and MUST include a copy of the legal decree.

Appointment of Custodian Form (and Custodian’s Consent to Appointment)

If a CFC Shareholder is less than 18 years of age, a custodian MUST be appointed. The shares the Shareholder is entitled to receive shall be held by a custodian (usually the parent or legal guardian) until the Shareholder turns 18 years of age. IMPORTANT: This appointment complies with Alaska Statute 13.46.085, as set forth in the Alaska Statutes Excerpts from the Alaska Uniform Transfers to Minor Act provided above.

Family Tree Form

A Family Tree is a visual history of your Native heritage. Fill in the individual branches and admire the tree as a whole. Start by filling in the Shareholder’s name, tribe, and blood degree. Next, add the name, tribe, and blood degree of the Shareholder’s parents and grandparents (both paternal and maternal).

Recipient Shareholder ID Card Form

New Shareholders must fill out a Shareholder ID Card Form to request a CFC Shareholder ID. A recent profile photo should be included with the form. If you do not have access to a profile picture, you can come to the CFC Saxman Office to get a photo taken. IMPORTANT: If you have received a gift of CFC stock, you must complete this form to receive your Shareholder ID Card

Shareholder Form (Gifting Shares)

A Shareholder who wishes to make a gift of shares MUST complete and have notarized a Shareholder Form. CFC does not accept Shareholder Forms by friends or relatives of the Shareholder. IMPORTANT: A gift of shares cannot be revoked, returned, or otherwise changed once made. This this affidavit must be signed in the presence of a notary public.

Testamentary Disposition Form

Each Shareholder MUST complete and have notarized a Testamentary Disposition Form which will allow CFC to follow your wishes in regard to who will receive your shares upon death. The Testamentary Disposition Form will act like a Will. Shares in CFC may be transferred by testamentary disposition using this form, a Last Will and Testament, or a Holographic Will. This form must be signed in the presence of a notary public.

W-9

Every new shareholder must fill out a W-9 form and include name, address, social security number, and signature. The remaining boxes are used for businesses and can be left blank.

Information Request Form

The Information Request Form can be used to request to review various business records for a limited purpose. The Information Request Form must state the information requested and a detailed purpose of request.

Affidavit of Address Form

If you are a NEW Shareholder of CFC, you MUST fill out an Affidavit of Address Form. Note that CFC does not accept Affidavit of Address Forms submitted by friends or relatives of the Shareholder.

Alaska Statues Excerpts from the Alaska Uniform Transfers to Minor Act

These legal statutes explain in detail the requirements of a CFC Shareholder under the age of 18 being appointed a custodian (usually the parent or legal guardian) until the Shareholder turns 18 years of age.

New Shareholder Checklist

Every Shareholder must have certain documents on file with CFC. Fill out the checklist form and include all required documents. Once completed, mail the form with the additional documentation to CFC.

Recipient Form (Accepting Shares)

Recipients who receive a gift of shares MUST complete and have notarized a Recipient Form. If the recipient is under 18, the custodian of the recipient (see Appointment of Custodian Form above) MUST complete and sign the form on behalf of the minor. On the back of the form, you can sign the optional Release for Publication. This grants the release of the recipient’s name in the Totem Times and other CFC publications. IMPORTANT: This form must be signed in the presence of a notary public.

Shareholder ID Card Form

To request a CFC Shareholder ID card, please fill out a Shareholder ID Card Form. Include a recent profile photo with the form. If you do not have access to a profile picture, you can come to the CFC Saxman office to get a photo taken.

Updated Shareholder ID Card Form

If you are gifting shares, you must fill out the Updated Shareholder ID Card Form. Include a recent profile photo with the form. If you do not have access to a profile picture, you can come to the CFC Saxman office to get a photo taken. IMPORTANT: If you have gifted any of your CFC stock, you must also complete this form to receive your updated Shareholder ID Card.

Principal Life Beneficiary Designation

Principal Life is CFC’s current life insurance provider. Principal Life provides assurance to our Shareholders and their families that their loved ones will be protected financially. Shareholders’ loved ones can use the benefit to cover final expenses, everyday living expenses, and long-term obligations that may otherwise cause financial strain. To ensure your loved ones are protected, complete the Beneficiary Designation Form to sign up for life insurance today.

Non-Disclosure Agreement

Company information is a valuable asset of the corporation, not available to the public. Cape Fox Corporation should protect this asset from unauthorized use and dissemination, including from other shareholders unless CFC has specifically approved that distribution/dissemination or any other form of communication. The Non-Disclosure Agreement must accompany the Information Request Form.
